114-1學期 博士班資格考試申請事宜

I.資格考申請路徑：校園入口網→學生資訊系統→各項申請→研究生資格考試申請

II.依指示填妥考試相關資料(詳參附件)上傳並列印繳至轉譯新藥所辦公室審核。

1.     博士學位候選人資格考試申請書 (紙本)
2.     歷年成績表 (紙本)
3.     博士學位候選人資格考核論文研撰計畫表 (紙本)

III.口試當天記得下列3項，並於口試完畢請繳交文件回所辦。

1.     領款收據（全部委員完成簽章）
2.     博士班研究生資格考試紀錄總表（全部委員完成簽章）
3.     博士班研究生資格考核成績表（全部委員完成簽章）

注意事項：

※應考條件：完成博士學位應修課程，成績及格者。
※博士班研究生須於修業三年內完成資格考核。
※114學年度第1學期研究生資格考核口試申請截止日：114.12.31。
※114學年度第1學期學位資格考核口試最遲於115.01.21前完成。

The first at all, you have to apply the Qualification Examination, please refer the attachment.

1. China Medical University Application form of Qualification Examination (apply from Student Information System of CMU website then print this application out with your advisor’s signature).

2. Transcript (upload)

3. Thesis Proposal (upload)

Please submit above all documents to office for further processing.

Remember the following 2 items on the day of the Qualification Examination, and please submit the documents back to the office after the oral examination.

1. Receipt of oral examination committee member including the ticket photo or ticket stub

2. The general evaluation form of the oral examination committee

※Test conditions: Those who have completed the required courses for a doctoral degree and passed the exam.

※Doctoral graduate students must complete the qualification assessment within three years of study.

※Deadline for application: 2025.12.31.

※Deadline for the date of Qualification Examination: 2026.01.21.
